Treatment

Your health information may be used by staff members or disclosed to other healthcare professionals for the purpose of evaluating your health, diagnosing medical conditions, and providing treatment.

For example, when providing treatment, Ismail MD may request that your primary care physician or other specialists share relevant medical information with us. We may also provide your primary care physician or other healthcare professionals with information about your condition so they can appropriately treat you for other medical conditions, when applicable.

In addition, laboratory test results, procedure results, and other relevant information may be included in your medical record and made available to healthcare professionals involved in your treatment or consulted by members of our staff.

Payment

Your health information may be used to seek payment from your health plan or other sources of coverage, such as an automobile insurer, or from payment providers you may use to pay for services.

For example, your health plan may request and receive information regarding dates of service, services provided, and the medical condition being treated.

Healthcare Operations

Your health information may be used as necessary to support the day-to-day activities and management of Ismail MD.

These activities may include quality assessment, administrative functions, care coordination, and other activities necessary to operate our practice and provide healthcare services.

Law Enforcement

Your health information may be disclosed to law enforcement agencies when permitted or required by law, including to support government audits and inspections, facilitate certain law enforcement investigations, or comply with government-mandated reporting requirements.

Public Health Reporting

Your health information may be disclosed to public health agencies as required or permitted by law. For example, certain communicable diseases may be required to be reported to the appropriate public health authorities.

Other Uses and Disclosures

Other uses and disclosures of your health information generally require your specific written authorization. If you authorize the use or disclosure of your information and later change your mind, you may submit a written revocation of that authorization.

However, revoking an authorization will not affect any use or disclosure that occurred before we received your written revocation.

You have certain rights regarding your protected health information under federal privacy laws. These include:

  • The right to request confidential communications concerning your medical condition and treatment

Practice Duties

Ismail MD is required by law to maintain the privacy of your protected health information and to provide you with this Notice of Privacy Practices.

We are also required to follow the privacy policies and practices described in this notice, as applicable under federal and state law.

Right to Revise Privacy Practices

As permitted by law, Ismail MD reserves the right to amend or modify its privacy policies and practices. Changes may be necessary to comply with updates to federal or state laws and regulations.

Upon request, we will provide you with the most recently revised Notice of Privacy Practices. Any revised policies and practices will apply to protected health information maintained by Ismail MD as permitted by applicable law.

Requests to Inspect Protected Health Information

You may generally inspect or obtain copies of the protected health information maintained by Ismail MD.

As permitted by federal and state regulations, requests to inspect or obtain copies of protected health information may be required to be submitted in writing. You may obtain a request form by contacting the office or the Privacy Officer.

Your request will be reviewed and generally approved unless applicable legal or medical grounds permit or require us to deny access.

Any applicable fees for copies of medical records will be charged in accordance with applicable federal and Florida law.

Marketing/Promotional

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.

All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Complaints

If you would like to submit a comment or complaint about our privacy practices, you can contact us to let us know. If you believe that your privacy rights have been violated, you should bring the matter to our attention by sending a letter describing the cause of your concern.
